Our webinar is designed for Administrators, Director of Nursing, and nursing staff that provide care to elderly and developmentally disabled persons. Our desire is to provide networking among leaders in the Post-Acute Care, Long Term Care, and Assisted Living industry while providing education on hot button issues. Jeff Nesheim, PharmD, MS
Senior Medical Outcomes Liaison
Jeff got his BS in Entomology from Oklahoma State University, PharmD from the university of Florida, and a masters in Pharmacoeconimics from the university of Florida. Jeff has worked in numerous positions within field medical affairs and health economics for pharmaceutical, device and diagnostic companies. Jeff lives in Littleton Colorado and enjoys skiing, cycling, hiking, and coaching and attending his sons sporting events.
Erin DuPrae, OTR/L
Clinical Support Specialist
Erin DuPrae has worked in skilled nursing and long term care for 21 years with 15 years spent in management and as a clinician at the facility level. After graduating from Eastern Michigan University with a Bachelor of Science Degree in Occupational Therapy, Erin has always enjoyed working with the geriatric population. Her efforts were directed toward returning each individual to his/her prior level of function, while incorporating humor and fun to improve quality of life. Erin joined Synchrony Rehab’s clinical support team in May 2015. She earned her certification as a Dementia Capable Care Instructor in 2016 and as a Dementia Capable Care Therapist (DCCT) in 2018. She has provided educational presentations on a variety of topics for healthcare professionals. Erin presented “Positive Therapeutic Approach to Successful Dementia Programming” at Ohio Health Care Association convention in 2017, “Modes of Skilled Therapy” with Vic Ramos at LeadingAge Indiana’s 2020 virtual conference, and “Non-Pharmacologic Management of Behavioral Symptoms in Dementia” at Kentucky Association of Health Care Facilities virtual conference in 2020 and at Ohio Health Care Association and LeadingAge Tennessee conferences in 2021. Erin supports therapy teams to assure compliance with documentation, to educate therapists of Medicare regulations, and to share her clinical knowledge. As a seasoned clinician with hands on experience working with people with dementia, and through professional growth and continuing education in the study of dementia and related disorders, Erin can provide insight for positive outcomes and increased quality care.
